Smokeless tobacco also poses health risks, researchers say

Secondhand exposure to tobacco may be possible even when no cigarette smoke is involved, according to a new study.

An analysis of dust samples from six homes with smokeless tobacco users shows that the dust there contains more tobacco-specific carcinogens and alkaloids than dust from non-tobacco-using homes, putting household members, especially children, at risk of indirect exposure

Scientists have long known that cigarette smoke is carcinogenic, whether inhaled by the user, by others as secondhand smoke, or brought into the home on clothing as thirdhand smoke.

But they didn’t know whether indirect exposures could arise with smokeless tobacco products, such as chewing tobacco or moist snuff.

Todd P. Whitehead of the University of California, Berkeley, School of Public Health and his colleagues wondered whether smokeless tobacco use at home might taint household dust.

They were already collecting dust samples from homes as part of the California Childhood Leukemia Study, which among other things explores how environmental exposures to contaminants such as cigarette smoke could influence leukemia risks.

Of the households they surveyed and analyzed, six told the researchers that they had users only of smokeless tobacco, six had users only of cigarettes, and 20 had no tobacco users.

Using liquid chromatography-tandem mass spectrometry, researchers analyzed dust samples from these homes for five nitrosamines that are found only in tobacco, as well as five tobacco-only alkaloids such as myosmine. The team also used gas chromatography-mass spectrometry to measure nicotine, the best-known tobacco alkaloid, which is addictive.

The team wanted to distinguish smokeless-tobacco contamination from cigarette-smoke contamination, so they calculated the ratio of myosmine to nicotine. In unburned tobacco, myosmine content is less than 0.5% of the nicotine content. But as tobacco burns, myosmine is produced, increasing the ratio to roughly 7% in tobacco smoke.

Levels of two key carcinogenic nitrosamines—N′-nitrosonornicotine (NNN) and 4-(methylnitrosamino)-1-(3-pyridyl)-1-butanone (NNK)—were roughly five and seven times as high, respectively, in the dust of smokeless-tobacco households as in samples from tobacco-free households.

Moreover, the median myosmine-nicotine ratio was 1.8% in smokeless-tobacco households versus 7.7% in cigarette-user households, confirming that contamination in the smokeless-tobacco homes came primarily from smokeless tobacco.

Whitehead warns that the researchers don’t yet know how much of a risk this dust contamination might pose, or how the smokeless tobacco got into the dust.

But the findings suggest that household members who don’t use smokeless tobacco could be exposed to its known carcinogens.

Young children, who often play on the ground and put their hands in their mouths, can accidentally ingest dust and may be especially vulnerable to exposure, he says.

The contamination could come from activities such as opening a package or dropping small amounts on the floor during use, Whitehead says, which could lead to simple strategies for reducing the risk of contamination.

Hugo Destaillats, an environmental chemist at Lawrence Livermore National Laboratory who wasn’t involved with the work, calls the findings an interesting first step.

The sample size was small, note both Whitehead and Destaillats, and the study didn’t directly measure anyone’s exposures.

Additional research, they suggest, could focus on the amount of tobacco-related compounds that young kids ingest when they live with someone who uses smokeless tobacco, and the resulting health risks. And Destaillats suggests doing future studies to see how dust contamination might vary in other geographical areas.

What's third-hand smoke? Researchers say it's far more dangerous than we ever realized.

Have you ever walked out of a building and smelled smoke even after the smoker has moved on? Or maybe caught a whiff on someone's clothes long after their cigarette has been extinguished? That's what scientists have termed - third hand smoke, and its effects may have been greatly underestimated.

Research led by the University of York has highlighted the potential cancer risk in non-smokers – particularly young children – of tobacco smoke gases and particles deposited to surfaces and in household dust.

Until now, the risks of this kind of exposure have not been well studied or considered in public policy.

However, a new study published in the journal Environment International, has estimated for the first time the potential cancer risk by age group through non-dietary ingestion and skin exposure to third hand smoke. The results indicate potentially severe, long-term consequences, particularly to children.

The research was carried out by York’s Wolfson Atmospheric Chemistry Laboratories, the National Centre for Atmospheric Science, and the Chromatography and Environmental Applications research group at the Universitat Rovira i Virgili, Spain.

The study demonstrates for the first time the widespread presence of tobacco related carcinogens in house dust, even in ‘smoke-free’ environments.

Scientists collected dust samples from private homes occupied by both smokers and non-smokers. Using observations of house dust composition, they estimated the cancer risk by applying the most recent official toxicology information.

They found that for children aged one to six years old, the cancer risks exceeded the limit recommended by the US Environmental Protection Agency (EPA) in three quarters of smokers’ homes and two thirds of non-smokers’ homes. The maximum risk predicted from the third hand smoke levels in a smoker occupied home equated to one extra cancer case per one thousand population exposed.

Lead investigator, Dr Jacqueline Hamilton, from York’s Wolfson Atmospheric Chemistry Laboratories, said: “The risks of tobacco exposure do not end when a cigarette is extinguished. Non-smokers, especially children, are also at risk through contact with surfaces and dust contaminated with residual smoke gases and particles, the so-called third hand smoke. This risk should not be overlooked and its impact should be included in future educational programs and tobacco-related public health policies.”

Experts Say Endocrine-Disrupting Chemicals Pose Global Health Threat

In an editorial published in Endocrinology, a journal of The Endocrine Society, endocrine experts agreed that endocrine-disrupting chemicals (EDCs) pose a threat to human health and to the ecosystems of the earth. The editorial comes in response to a commentary (Dietrich et al. Chem Biol Interact) signed by a number of editors of toxicology journals that dismisses the state-of-the-science on EDCs and argues for the status quo in the regulation of these hazardous substances.

EDCs are commonly found in food and food containers, plastic products, furniture, toys, carpeting, building materials, and cosmetics. They are often released from the products that contain them and enter the bodies of humans and wildlife through dust or through the food chain. A large volume of studies have shown that EDCs exert their effects by interfering with endogenous hormone action and can impact male and female reproduction, breast development and cancer, prostate cancer, neuroendocrinology, thyroid, metabolism and obesity, and cardiovascular endocrinology.

“The Dietrich et al. paper neglects the fundamental principles of how the endocrine system works and how chemicals can interfere with its normal function, nor does it consider the consequences of that interference,” said Andrea Gore, PhD, lead author of the editorial and Editor-in-Chief of Endocrinology. “We cannot have sound policies for regulating these chemicals when we ignore the science.”

The Endocrinology editorial, Gore et al., represents an unprecedented response from the endocrine community. The editorial was signed by 20 editors-in-chief and 28 associate and senior editors of endocrine, neuroendocrine, environmental, and other peer-reviewed journals. Dr. Teresa Woodruff, PhD, President of The Endocrine Society and several other presidents of societies or medical organizations have also signed on to the editorial.

The Endocrine Society has published a Scientific Statement on EDCs—a thorough review of the literature, with recommendations for improved policy—and a Statement of Principles—a summary of critical endocrine principles that are relevant to risk assessment as it applies to EDCs. In its Statement of Principles, the Society recommends that endocrine principles be incorporated into programs by the EPA and other agencies charged with evaluating chemicals for endocrine-disrupting potential.

“Regulatory decisions on the use of EDCs should be made based on the best available science and expertise that involves among others, reproductive biology, endocrinology, medicine, genetics and toxicology,” said Gore.
